Insurance Verifications

We call your insurance company to obtain a breakdown for a specific service to help you identify what services are covered and what services are not covered. Verification of benefits is not a guarantee of payment and claims are subject to the plan provisions and eligibility at the time services are rendered. 

GAP Exceptions/Single Case Agreements

GAP Exceptions/Single Case Agreements is a request for insurance to honor a patients in-network benefits, even though they are seeing an out of network provider. Once the GAP Exception is received, we will request a single case agreement to negotiate a higher reimbursement rate.

Superbill Request

We will provide you with a customize superbill to provide your insurance for reimbursement of services you have paid for that have not been billed out to insurance.

Appeals

Receiving a denial for services rendered can be very frustrating. We at Katmere will provide you with guidance and assistance in appealing denied claims and work with you on writing an appeal to submit to your insurance to have claims paid.
